Important information for dental offices affected by Hurricanes Ike and Gustav
Update: Texas has issued multiple Bulletins regarding Hurricane Ike and the state of emergency/disaster proclamation that has been issued for the 33 counties affected in effect until October 12th. The counties affected are: Angelina, Austin, Brazoria, Brazos, Chambers, Cherokee, Fort Bend, Galveston, Gregg, Grimes, Hardin, Harris, Houston, Jasper, Jefferson, Liberty, Madison, Matagorda, Montgomery, Nacogdoches, Newton, Orange, Polk, Sabine, San Augustine, San Jacinto, Rusk, Trinity, Tyler, Walker, Waller, Washington, and Wharton. These bulletins apply to the residents/companies in these counties regardless of where the individuals have been relocated.
We at Delta Dental want to make every effort to help dental offices that were affected by Hurricane Ike or Hurricane Gustav.
If for any reason you have had to relocate, and your address and phone number have changed temporarily, please let us know by e-mailing us at prsoutheast@ddic.delta.org or by calling (800) 521-2651, ext. 5130. Your notification will be effective for Delta Dental Insurance Company, Delta Dental of California, Delta Dental of Pennsylvania and Delta Dental of New York. If you need us to share your temporary contact information with other Delta Dental member companies, please let us know which one(s).
We will work to ensure that you receive any claim payments for services you submitted before the hurricane, as well as future claims you want sent to your temporary address.
If you lost patient records as a result of the hurricane, we can offer Delta Dental’s assistance in recreating them. We would be happy to provide you with copies of claims you have submitted to us for the past several years. Just let us know if and when you would like this kind of information.
